Location: Sawley Medical Centre / Littlewick Medical Centre

The post holder may be required to work at any establishment at any time throughout the duration of their employment, normally within the location of the Erewash PCN.

About Erewash Health Partnership (EHP)

Erewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of nine constituent practices (see below). EHP employs approximately 220 people who work within the constituent practices and central team.

As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.

EHP is governed by the EHP Senior Leadership Team reporting to the EHP Board which meets quarterly. The Board currently consists of one GP Partner from each constituent practice and the EHP Managing Partner & CEO, with other colleagues in attendance.

Constituent Practices

EHP consists of 8 GP practices who are contracted to deliver primary care services to their registered patient population through a GMS or PMS contract. The practices range in size from approximately 3,500 patients to 19,000 patients.

Job Summary

This is a generic Job Description for Advanced Clinical Practitioners (ACPs) working within EHP. Specific responsibilities will vary between specialities; however all ACP posts will be expected to reflect the four pillars of advanced practice: clinical practice, leadership, education of self and others, and research and development.

You will work independently, making professional autonomous decisions. The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluations of patient care.

You will work to a high level of personal and professional autonomy, using specialist knowledge and skills to deliver independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, health promotion and safe admission and discharge of patients.

The role will involve independent prescribing within the scope of professional practice.

Work collaboratively with all members of the multidisciplinary team to provide best clinical care, emotional and psychological support and education to patients and their carers.

General duties

At all times the post holder is to behave in a professional way that encourages quality care, the development of a team spirit and fosters continual improvement across the PCN.

The post-holder will be expected to carry out any other duties that may reasonably be required or directed in line with the main duties and level of the role to meet the changing needs of EHP and EPCN.

Key Job Responsibilities

Clinical

• Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (National) and local clinical guidelines and local care pathways). With moving and handling of patients as required.
• Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
• Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
• Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
• Maintain competence in clinical skills required to practice at an advanced level in own professional discipline, and to acknowledge limitations.
• Be responsible for making any necessary referrals, requests for investigations and do these in an appropriate manner and timescale.
• Draw on own specialist knowledge to be a resource for the wider multidisciplinary team (MDT).
• Reflect on own clinical practice in individual and peer supervision. Responsible for own professional actions and recognise own professional boundaries, interpreting and applying clinical and professional policies and national protocols.

Communication

• Maintain accurate records regarding all patients contacts on Systm1.
• Receive/provide highly complex and sensitive information relating to patient care.
• Communicate clearly, concisely and accurately, engage patient and colleagues with tact and empathy, and interact with sensitivity with patients, relatives/carers where barriers to communication may exists (this may involve using interpreting services for patients with language barriers).
• Provide written and verbal reports to other members of the multidisciplinary team regarding treatment/changes and recommended management of patient care. Communicate results of tests to patients, relatives and other clinical staff.
• Participate effectively in multidisciplinary case conferences and team meetings, as required. Work to manage conflicting views and reconcile differences of opinion across/within professional groups.
• Attend relevant internal/external meetings e.g., to discuss the performance of the service and/or promote the achievements of the service.

Leadership

• Champion the development of own speciality, opportunities for research and education, and raise the positive profile of the service and EHP through, for example, publications, conference presentations, professional networking and organisational collaborations etc.
• Act as a professional role model, providing advice and support to all staff working in the service, including medical and other students / trainees, nurses and doctors.
• Demonstrate the EHP values and behaviours in own day to day work, reinforcing positive behaviours that reflect our values, and challenge undesirable behaviour constructively and with positive intent.

Training and Development

• Keep up to date with current research and evidence based practice by attending relevant meetings, courses, seminars and own literature study. Share knowledge with other professionals and team to influence better care and outcomes for patients.
• Use advanced specialist clinical knowledge to design and provide clinical and theory based education and training to other members of the MDT and wider partnership.

Professional development

• Accountable for own practice and maintaining competence in accordance with the HCPC/GPC/NMC codes of practices and appropriate guidelines and statutory guidelines.
• Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities, and provide evidence of learning activity as required.
• Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety etc.
• Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
• Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
• Attend relevant education and training, including mandatory and statutory training.

Miscellaneous

• Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner.
• Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning.
• Work in accordance with the EHP, PCN practice’s policies and procedures.
• Contribute to the wider aims and objectives of the PCN to improve and support primary care.

• In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include:

• Using personal security systems within the workplace according to practice guidelines.

• Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.

Flexibility

Due to the still emerging nature of Primary Care PCNs and EHP the nature of this post will undoubtedly change. As such, flexibility is essential. The postholder is required to carry out any duties that may reasonably be requested by the Partners, Clinical Directors or Senior Leadership Team.
