Permanent contract | £27,344 to £29,439 per annum (Scale G) | 37 hours per week | Norwich Community Hospital
An exciting opportunity has arisen for Assistant Practitioners to join our Home First Hub. The Home First Hub is an integrated team working closely with our Norfolk Community Health & Care (NCH&C) and NHS colleagues to ensure safe and timely hospital discharge for those in community and acute hospitals across the Central area. The Home First Hub works within the Discharge to Assess model and is a new and developing team committed to supporting people to return home, access
reablement opportunities and plan their ongoing care and support needs once discharged. The Home First Hub workers currently work from community hospital settings and remotely. We are looking for a longer-term base to continue to develop this dynamic integrated team. You’ll have experience in the Health or Social Care sector, have a good standard of education, and have excellent IT and organisational skills. You will be competent at working at pace in a responsive fast paced environment. A positive can-do attitude is essential alongside professional curiosity around information exchange and community resources. You will be capable of working independently as well as part of a team. You will have good listening and analytical skills, be flexible and adaptable to new ways of working and be committed to supporting people to leave hospital safely. Some weekend working will be required.
This is an exciting time to join the Home First Hub as we are a developing service. We will expect committed performance from you but will offer a strong induction, the opportunity to learn new skills and a supportive working environment. While this post is for the Home First Hub and is largely remote / office based this team is part of a large-scale transformation around the development of the Discharge to Assess model. This means that in line with service need and development your role may stay in the Home First Hub or move to assessing people in their places of residence in the community. This review is to optimise the experience for our service-users/patients. At this stage it is not clear whether or how this may impact on the operating model in the future, however, we will continue to engage with Hub members about any changes as appropriate over the coming months.
