An opportunity has arisen for an enthusiastic, motivated individual who is committed to improving outcomes for families in Torbay to join the Children with Disabilities Team as a Community Care Worker.
As a Community Care Worker in this team, you will have the opportunity to make an impact on the lives of children, young people and their families – this is a highly rewarding position which will support your personal and professional development, widen your horizons and enable you to build a wealth of skills and experience.
In this role you will be a key part of the Team, holding a workload of allocated children who have been assessed as requiring a package of support and care as a result of their disability.
You will undertake visits and hold review meetings to ensure children’s current needs are reflected in their package of support.
You will work with partner agencies in health and education to review and adapt plans to meet the needs of children and their families.
You will be working alongside a team of skilled and experienced Social Workers and Community Care Workers who will guide and support your practice.
