Adults Social Work Assistant / Social Care Officer

Unqualified Adults Social Worker / Practitioner required to help with care reviews, ideally based close to or happy to commute into West Midlands. Hybrid-working opportunity.

This role will require you with work with adults with many different needs.

You will be working on a hybrid basis and your role will involve assisting with;-

Providing an assessment of the needs of individual service users and their carers.
Arranging services from existing resources to help meet the identified needs of individual service users and their carers and to monitor and review progress.
Understanding by the needs of each case and the policies of the department to provide the best possible service to service users.
Providing practical and emotional support to patients and carers through individual work and groupwork.
Promoting good quality interagency relationships through effective liaison with medical, nursing, and paramedical staff.
Assisting with care reviews, 5-8 per worker per week to be done face to face.

Pay rate of £25 an hour.
