A new and exciting opportunity is available to join a multi-disciplinary team, in the role of Optimising Independence - Home First - Senior Healthcare Assistant within the Hull & East Riding region. The role is graded as Agenda for Change Band 3 and involves supporting people with health and social care needs, within their own homes to regain their independence and manage their health and care needs.
