Team Leader– Sittingbourne

A fantastic opportunity to work as a *Female Team Leader supporting an 18 year old young woman in Sittingbourne has arisen. You would be working in a fully adapted accessible home with a wonderfully supportive rehabilitation therapy team, care team and the client’s family.
The case management company are looking for a team leader to work closely with the family and to support an established care team. They should be client focused, creative, caring, reliable, proactive, flexible, and hardworking with a positive and engaging energy. With the focus of enhancing the clients day to day living at home and in the community.
Working as a team leader, you will provide 2-to-1 care when on clinical day shifts and 1-to-1 if required to work night shifts. Participation in therapies, hydrotherapy, planning activities and facilitating days out and accompanying family holidays will be required along with regular driving of the wheelchair accessible vehicle.
You will need to complete various admin tasks such as; coordinating appointments and therapist visits, ordering stock and supplies including medication and liaison with the GP/community nursing teams, managing the care team rotas, timesheets and training. Completing client specific paperwork including care planning and on-call support is sometimes required to support with staffing and client health issues.

Rates
Monday to Friday: £18.00 per hour
Weekends: £20.00 per hour
Sleeping night: National minimum wage
Bank Holidays: 1.5 x the weekday hourly rate
Shift Times
Shifts will include days, nights and admin hours. This is on a rota basis totalling 40 hours per week Monday to Sunday.
The Client
18-year-old girl who suffered an acquired brain injury. She is non-verbal and a full time wheelchair user. She needs full support with her day-to-day personal care, nutrition, mobility, therapy and psychological needs. She loves being outdoors, going shopping and watching live shows/TV. She enjoys hydrotherapy, her trike, music, loves animals and has a fun, happy personality.
Person Specification
· Familiar with handling equipment to ensure the client can access the full range of resources which is crucial in her day-to-day life;
