Norwich Integrated Care Coordinator Team Leader (Planned Care)

An exciting opportunity has arisen within the Integrated Care Coordinator (ICC) team for Norwich for a permanent and full time Team Leader. You will lead a team of ICC’s, working in the Norwich area, who are hosted by Norfolk County Council but work in partnership with other system colleagues. You will work alongside many different organisations and professionals such as Norfolk Community Health & Care (NCH&C), General Practice, Housing colleagues, Voluntary sector, ICB colleagues and Norfolk County Council, fostering and promoting multi-agency working. The team includes ICC’s working across services such as Community FICS (which will be the largest part of the role), but you will also line manage ICCs in other integrated services.

Community FICS ICCs are aligned to General Practice and wider multi-disciplinary teams, supporting colleagues to work closer together, retain a focus on prevention and early support and long-term condition management. You will support the ICCs with daily tasks such as monitoring and tracking the person journey and support the team to manage a variety of situations using data, local relationships, knowledge to connect people to their communities. You will work with multiple systems such as LAS, SystmOne and NCAN. You will be positive, flexible, and embrace change. You will form close working relationships with other ICC services across Norfolk, working specifically with colleagues in community and GP settings as you support the development of Integrated Neighbourhood Teams.

You will be a highly motivated and enthusiastic candidate and will have a high level of education (eg NVQ level 3, GCSE Levels or equivalent) and previous experience of working in voluntary sector, primary care, the NHS or Social Care. A positive can-do attitude is essential alongside professional curiosity around information and community resources. You will have an awareness of relevant health and social care policy, excellent interpersonal and organisational skills along with a good level of administrative and IT skills.

The services operate a hybrid model of working remotely and office based, you will be required to spend time in other bases around the Norwich area as needed. We offer a friendly, supportive team, regular supervision, and the chance to use operational and transformation skills as we continue to develop service improvements.
